Kathie Lee Gifford made a pretty huge blunder Wednesday morning on Today when she asked Madagascar 3: Europe's Most Wanted star Martin Short about his wife, Nancy Dolman. The problem? Dolman died of ovarian cancer in 2010.
Still, Gifford isn't alone; here are five other TV interviews that had us covering our eyes, pressing the mute button on our remotes and/or left our jaws on the floor:

1. Megan Fox: The pregnant star's interview with ET's Steve Jones turned sour fast when the former X Factor host did something he wasn't supposed to: ask about her pregnancy! "Do you know the ultimate gadget that every man wants?" 3. Vanessa Hudgens and Josh Hutcherson: Before shooting to superstardom thanks to The Hunger Games, Hutcherson and Hudgens dated while filming their adventure movie Journey 2: The Mysterious Island. Their romance fizzled before the film came out, but an Australian talk-show anchor did not get the breakup memo, which led to one of the most embarrassing silences ever caught on film when the interviewer asked how long the duo have been dating. Cue long, awkward pause. "We're not," Hutcherson said, finally breaking the tension, as Hudgens shook her head. "We were at one point, but she broke my heart…no, I'm just kidding." Yeah, it's pretty painful.
